The agreement was signed by three unions (FO, Unsa, CFDT), the Hospital Federation of France and three ministers (Olivier Véran, Minister of Health, Adrien Taquet, Secretary of State in charge of children and families and Amélie de Montchalin, Minister of Transformation and the Public Service). But what it contains does not satisfy all the trade unions. The majority union of the profession, the national trade union organization of midwives (Onssf) thus denounces an agreement signed by “trade unions which have almost no midwifery members or who sign when their members are not. disagree”.
Insufficient salary increase
The Minister of Health announces a salary increase of 500 euros net per month for all hospital midwives, whether permanent or contractual. “It is unheard of in any category of professionals. It is historic,” greets Florence Dedieu, national secretary of the CFDT Social Health. Onssf, for its part, deplores in a press release that this amount is “much lower than the recommendations of IGAS”, of 624 euros net monthly.
The National Council of the Order of Midwives warns him about “fragmented information,” because in the 500 euros announced, are included the 183 euros from the Ségur de la santé and intended for all caregivers. “This is not a gesture for midwives,” said Isabelle Derrendinger, secretary general of the National Council of the Order of Midwives.
A “medical exercise premium” of 240 euros is also included within the 500 euros. “But this is accompanied by the abolition of the premium for paramedics, which was 90 euros gross, so we rather fall to an increase of 150 euros”, warns Isabelle Derrendinger. It also adds that “the provision is aimed at a third of the profession and does not concern everyone”. The medical practice premium does not actually apply to midwives in the territorial function or to those working in maternal and child protection (PMI). Liberal midwives are not affected either.
No mention of the workforce
“Unfortunately, the account is still not there, regrets Camille Dumortier, president of the Onssf. This agreement will not make future students want to join the profession or the midwives in post to continue to exercise it. “. The profession demanded a review of the perinatal decrees, which date from 1998, in order to be able to gradually increase the workforce. “But this request has remained a dead letter, while these decrees must imperatively and urgently be revised”, considers Isabelle Derrendinger.
The government has undertaken to publish by the end of the year an instruction on organizational and working methods at the hospital (HR management by the Medical Affairs Departments, access to continuing training, role and place of maieutics coordinators…). “These are advances, midwives will find a place within the hospital public service which was not really asserted until then”, considers Florence Dedieu (CFDT Social Health), when Camille Dumortier (Onssf) calls for put midwives “in the right place” so that they “participate in the governance of hospitals”.
A new “black weekend”
The government has also agreed to extend the training of midwives by one year, which will therefore increase from five to six years from the 2022 class. “This will allow all the new skills that midwives do to be integrated and thus to recognize them “, explains Florence Dedieu. “It is fundamental to reorganize the course with a sixth year in boarding school if possible. But to be paid 2300 euros including nights and weekends with a Bac + 6 is not enough”, warns Camille Dumortier. Finally, the agreement provides for double promotion rates until 2024 for midwives working in hospitals.
If the government, through the voice of Oliver Véran, considers these advances “major”, they should not make it possible to stop the mobilization underway since the beginning of the year. The CGT and the CFTC filed last Thursday a national strike notice for this weekend. The Onssf calls for a “new black weekend on November 26-27-28-29” and adds that “if that is not enough, we will organize a black week from December 24 to 31”.
“It’s a sensitive and really complicated issue,” sighs Florence Dedieu. “As long as the entire profession is not recognized at its fair value, we will not stop, promises Camille Dumortier. The children and citizens of tomorrow, we are today obliged to welcome them in unacceptable conditions. . We opened our eyes, we will not close them “.
